Complete guide to surviving extreme heat waves in Germany: what risks are real, who is most vulnerable, and the practical measures that can save lives during extreme summer temperatures.

Why Heatwaves Are Underestimated

Heat kills more people in Europe than floods, storms, and earthquakes combined. The European heatwave of 2003 killed approximately 70,000 people across the continent. Germany's 2019 and 2022 heatwaves each killed thousands. Yet most households have no heatwave preparation plan.

The reason heat deaths are underestimated: they look invisible. Heat doesn't destroy buildings or create dramatic images. Most heat deaths are elderly people who die alone in poorly ventilated apartments, their deaths attributed to heart failure or respiratory problems — not to the 38°C temperatures outside.

Germany is now experiencing extreme heat events that would have been exceptionally rare 30 years ago. Temperatures above 40°C have been recorded. The trend is clear: heatwaves are more frequent, more intense, and longer.

Who Is at Highest Risk

Understanding vulnerability is the first step to targeted preparation.

Highest risk:

  • Elderly people (65+): reduced ability to sweat effectively; often on medications that further impair thermoregulation; reduced thirst perception
  • Infants and young children: high surface-area-to-body-mass ratio; cannot communicate distress; cannot self-regulate
  • People with heart disease, diabetes, kidney disease, or respiratory conditions
  • People on medications including diuretics, antihypertensives, antipsychotics, anticholinergics (many common medications impair heat tolerance)
  • People without air conditioning in urban areas (urban heat island effect adds 3–8°C to actual temperatures)
  • Outdoor workers and athletes during peak heat hours

If you have elderly relatives or neighbors: They are potentially at life-threatening risk during heatwaves. This is not an exaggeration. Check on them daily during extreme heat. Offer to bring them to a cooler location.

Preparation Before a Heatwave

Assessing Your Home

Temperature mapping: Walk through your home during a hot day and identify which rooms stay cooler. South- and west-facing rooms heat most severely. Interior rooms and north-facing rooms stay cooler. Know your coolest space.

Insulation of heat:

  • Solar-reflective exterior blinds or shutters: the single most effective intervention. Exterior shading prevents heat from entering through glass. Interior blinds help but are less effective (heat already enters).
  • Heavy curtains on south and west windows
  • Sealing gaps where hot air can enter

Equipment to Have Ready

Active cooling:

  • A portable electric fan: moves air, aids evaporative cooling from sweating. Effective up to approximately 35°C. Above that, it actually increases heat stress by moving hot air over your skin.
  • Spray bottle with water: misting + fan significantly increases cooling effect through evaporation
  • Bucket or tub of cold water: soaking feet dramatically reduces core temperature (arterial blood supply near the surface)

Passive cooling:

  • Wet towels in the freezer: apply to neck, wrists, and ankles where blood vessels are close to the surface
  • Frozen water bottles: in bed for sleeping; applied to pulse points
  • Blackout curtains or exterior blinds

Monitoring:

  • Indoor/outdoor thermometer: knowing actual indoor temperature helps make rational decisions about when to leave for cooler spaces
  • Hygrometer: humidity above 60% significantly impairs evaporative cooling

Medication Review

If you or a household member takes medication, ask your doctor or pharmacist before a heatwave: does this medication affect heat tolerance? Common medications that increase heat risk include:

  • Diuretics (increase dehydration risk)
  • Beta-blockers (impair heart rate response to heat)
  • ACE inhibitors and calcium channel blockers (blood pressure regulation)
  • Anticholinergics (impair sweating)
  • Antipsychotics and some antidepressants (impair temperature regulation)

Know which medications your household members take and whether dose timing or adjustments may be warranted. Do NOT stop medications without medical guidance.

During a Heatwave

The Critical Temperature Thresholds

Up to 30°C (86°F): Normal summer heat. Hydration and avoiding peak sun hours sufficient for healthy people.

30–35°C (86–95°F): Heat stress range. Extra hydration, limit outdoor activity, close windows during the day, open at night.

35–40°C (95–104°F): Dangerous range for vulnerable people. Check on elderly relatives. Avoid outdoor activity between 11 AM and 5 PM. Cool indoor spaces are essential.

Above 40°C (104°F): Extreme danger. Air conditioning or a public cooling center becomes a health necessity for vulnerable people.

Hydration

In extreme heat, adults need 3–4 liters of fluid per day minimum. More with physical activity.

Key points:

  • Thirst is a late indicator of dehydration — drink before you feel thirsty
  • Water and electrolyte-containing drinks (not sugary sodas) are best
  • Avoid alcohol (diuretic effect)
  • Check urine color: dark yellow indicates inadequate hydration; pale yellow is the target

Elderly people often have reduced thirst perception and may become severely dehydrated without noticing. Remind elderly relatives to drink regularly — set hourly reminders if needed.

Cooling Strategies

Most effective body cooling techniques:

  1. Immersion in cool (not cold) water: bath, lake, pool, fountain
  2. Cool wet towel on neck, wrists, armpits, groin (high blood supply areas)
  3. Fan combined with wet skin: most effective active cooling without air conditioning
  4. Cool shower or foot bath

Behavioral:

  • Sleep during the hottest parts of the day if possible
  • Move to the coolest room in the home
  • Avoid cooking indoors (adds heat to living space) — eat cold foods
  • Open windows only at night when outside temperature drops below indoor temperature

Public Cooling Resources

In German cities, public cooling resources include:

  • Public swimming pools
  • Air-conditioned libraries and museums
  • Shopping centers
  • Some cities operate designated "Kühlräume" (cooling rooms) during extreme events

Heat Emergency Recognition

Signs of Heat Exhaustion (Medical Attention Soon)

  • Heavy sweating
  • Cold, pale, clammy skin
  • Fast, weak pulse
  • Nausea or vomiting
  • Headache, dizziness
  • Weakness, fatigue

Action: Move to cool place; drink water; apply wet cloth to skin; call 112 if no improvement within 15 minutes.

Signs of Heatstroke (Medical Emergency — Call 112)

  • High body temperature (39°C+)
  • Hot, red, dry or damp skin
  • Rapid, strong pulse
  • Confusion, slurred speech, loss of consciousness

Heatstroke is life-threatening. Call 112 immediately. While waiting: cool the person by any means available (ice water, fan, wet cloths). Do not leave them alone.

Heatwave Preparation Checklist

Equipment:

  • Electric fan + spray bottle
  • Exterior blinds or blackout curtains on south/west windows
  • Indoor thermometer
  • Frozen water bottles (at least 6 in freezer)
  • Know location of nearest public cooling space

People:

  • Identified all elderly/vulnerable people in your vicinity
  • Have a plan to check on them daily during extreme heat
  • Reviewed medications with doctor for heat-related risks

During a heatwave:

  • Drinking 3–4 liters of fluid daily
  • Keeping one room below 25°C
  • Checking on vulnerable neighbors daily
  • Knowing the signs of heat exhaustion and heatstroke
